Also known as: Distinct procedural service (and the X{EPSU} subsets)
Dermatology uses it constantly because multiple lesions are treated in one visit. A biopsy of a lesion on the back and a destruction of a separate lesion on the forearm are distinct services even though the code pair carries an edit.
Many payers now prefer the more specific X modifiers — XE, XP, XS and XU — which name the reason the services were distinct. XS, separate structure, is the one that fits most dermatology scenarios.
Why it matters to your revenue
Missing it bundles legitimate work into a single payment; overusing it is a classic audit trigger, especially paired with modifier 25.
